Medicare Beneficiaries: Get
Replacement Cards Online
People who get Medicare
benefits need a Medicare card to show to medical treatment sources to obtain
services. The Social Security Administration (SSA) currently processes
about 750,000 requests per year for replacement cards for beneficiaries.
For convenience, SSA has created an online form that beneficiaries can
fill out to request replacement cards. To use the form, visit www.ssa.gov/medicarecard.
You will need the following information:

Your name as it appears on your most recent Social Security Card;

Your Social Security Number;

Your date and place of birth;

Your mother’s maiden name (to help identify you);

Your phone number in case SSA needs to contact you about your request;

Your e-mail address (optional);
-
Your last payment amount or the amount and year you last received a payment
if you have received benefits in the last 12 months.
